Cannot reproduce on OOo 2.0.3FR Mac OS X 10.3.9 PPC.
I tried both with your example and with a new one that I created, and the Form
and Report buttons are both active and working.
Did you tried with another set of data? Can you precise what OS you are using?
Are you using a localised version of OOo?
