This patch removes a chunk of obsolete #if defined(MAC) logic, and merges an
OS/2 conditional into a Unix one.

The question as to whether on modern Unix systems there is any point in holding
on to 1/2 a Mb of memory, so we can try freeing it as/when -one- type of
allocation fails is left open :-)

Mathias - does your custom allocator honor set_new_handler ? [ I imagine a lot
of the libraries we link to don't handle malloc failure correctly either way 
;-].
