Using OOo 2.1,

The issue could be exactly reproduced by following the steps mentioned above.  
It should be noted that slide renaming can be done in any of the “Normal”,
“Notes”, or “Slide Sorter” views.

Once the name of a slide is changed from its original name, (which consists of
the word “Slide”, a space after that, and the number of the corresponding slide
following the space) to some other name, the new name cannot be changed back to
the original name.  In other words, the name cannot be changed back to the
format consisting of the word “Slide”, a space after that, followed by the slide
number.
When this format is followed in renaming, the “ok” button is grayed out not
letting you choose this type of name.  However, the name can be changed to any
other name in any other format.

The work-around for this problem is:
-  Copy all the content of the slide by selecting everything and right clicking 
 
   and choosing “Copy”
-  Delete the slide by right clicking on the slide in the “Slides” bar which is 
   located on the left hand side of the page and clicking on “Delete Slide”.
-  After the slide has been deleted, on the “Slides” bar on the left hand side 
   of the page, put the cursor where you want to have that slide and right click
   choosing “New Slide”.  
-  The new slide created has the desired name which consist of the word “Slide”,
   a space after that, and the number that corresponds to the number of the 
   slide.
-  In the slide (not on the “Slides” bar on the left, but in the slide itself) 
   right click and choose “Paste”.  This will copy the content of the previous 
   slide which could not be renamed to the desired format.  Therefore you have 
   the same slide with the desired name format.
