In the header type "Mark 7", and center it. In the footer type "Page", enter Insert -> Fields -> Page Number, type <space>of<space>, enter Insert -> Fields -> Page Count, and center it. Print to the HP Laserjet Series II driver (standard Win2K built-in driver). I am using a QMS-410 printer, using the HP emulation. The "o" in the word "of" in the footer is not printed.
