2007 -------
Confirmed in m207 on win XP
Create doc with first page with 'textx'
do insert > manual break > page break twice
on last page put in a title, assign to Heading 1
on previous empty page, do insert >  .. > table of contents
This document has three pages, with index on second page
Save document (doesn't have to be a template)
Close document
Reopen document
Resulting document has only two pages, with 'textx' followed by the index on the
first page

Reported behaviour with a small frame centered on first page makes the index
appear before the frame.
