There have been some bugfixes with chapter numbering in the meantime. I have
opened file "Problem_1.odt" with German OOo2.2 and the TOC is OK after 'update
index'. Please use the actual version of OOo and see if it is OK for you now.
@xavidp: Your file is a different problem. You have mixed up list numbering and
chapter numbering. Remove the list numberings on the headings totally and use
Tools > Outline Numbering.
