So this is correct. second PNG: Could not reproduce. Maybe you have a broken Userprofile, move it away and create a new one. Does it still occur ? third PNG: Nothing strange fourth PNG: Basically a Chart is an OLE Object if not the most used OLE object, so the Icon on the toolbar (Chart) and the Icon in the Visible Buttons menu are correct.
