Preliminary note: this issue has been encountered on a french version of Ooo
2.3.1. I do not know about the the english version.

When a title level has been given a numbering style (with the style editor
dialog), the corresponding entry level in the index disappear. And it is not
possible to go back and show again this title level by removing the numbering 
style.

Steps :

- Create a new document
- Create a few titles :
Test 1 [Title 1]
Test 2 [Title 2]
Test 3 [Title 1]
etc.

- Create the index : all the titles are shown in this index
- From the Styles window, modify the "Title 1" style (it opens a dialog), go to
the "Numbering" tab, and select a numbering style in the drop-down list.
Note: when this dialog is shown and no change has been done yet, the drop-down
list displays an empty item. This empty item choice is not available in the
list. When a change has been done to this property, it is not possible to go
back to this state, the available choices are "none", "numbering 1", etc.

- Update the index: all the entries for the level 1 titles disappear, and it is
absolutely impossible to get them back.

Note: if the numbering is created through the menu item "bullets and numbering",
the index is updated correctly (provided that the above action has not been 
done).
