2008 -------
With an extension it is possible to merge new items into an existing menu or 
toolbar.
But the changes are not reflected in the personalization mechanisms.

Merge some buttons in a toolbar - see attachment mergeBtn1.gif
Open Tools > Personalize > Toolbar tab. These buttons do not appear, see 
attachment

Open the "Visible buttons" toolbar menu : the added buttons are here, but the 
icons 
are not displayed, see attachment mergeBtn3.gif.
Furthermore, it is not possible to switch off the visibility of those buttons.

Merge some items in a menu, see attachment mergeMenu1.gif
Open Tools > Personalize > Menus tab. The new items are not displayed at all, 
see 
mergeMenu2.gif.
