The document contains page bound frames. This is not supported in mail merge as
it is not (easily) possible to find the right position of such frames in the
appended pages.
The duplication of graphics depends on the fact that they are bound to frame.
The actual bug needs more investigation.
I've added a modified document where the frames and graphics are anchored to
paragraph. This way the mail merge works.
