I have had this problem a lot as well. I frequently use documents with a lot of
formulae in and so this becomes quite frustrating. paulgi said that writer had
crashed initially, though my writer hadn't crashed and yet the formulae (in my
own documents as well) still got mixed up.

Note that there's no way to recover the formulae that have been replaced
(perhaps there's a way by delving into the xml and salvaging bits but your
average end user isn't going to do that). This is a big problem with large
formulae (which I use quite a lot) as they have to be re-entered quite often.
