Solution: don't "Edit the style of the first level in the TOC: Text effects:
capitalize."
Instead:
- On tabpage "Entries" of the Index dialog, click [E]
- choose a character style in the listbox
- click "Edit"
- set here "Capitals" under "Font effects"
