The problem is that you use hard formatted numbering to build a document
outline.
Don't use "Format - Numbering/Bullets - Outline" to apply to heading but define
the numbering for each level under "Tools - Outline numbering"
The numbering is then applied as style and doesn't change.
When you pressed Enter to type "B" you broke the list formatting and reset the
numbering to 1.
