There is still a problem here which is that if I have a document that has been
created as I described, then I can't fix it to make the numbering follow the
expected sequence.
As far as the user is concerned, this is broken behaviour: how is the user
expected to know that one approach causes 'painted numbers' whereas the other
approach results in a 'real outline numbering structure'. Especially given that
there is an 'outline numbering' tab in the same place. That tab should perhaps
be taken away, or be renamed?
