You have to manually add it, even when the "Auto Extension" box is checked. This seems to happen when using Writer, Calc, etc, as well as when exporting as pdf. Additionally, there is another file generated in the folder where the (above mentioned) malformed file is stored: while OOo is open and running, a '.~lock.Filename#' appears, until you close that particular file (or OOo altogether).
