2008 -------
Trying to insert a new sheet with name=Foo will fail with "The table could not
be inserted" if:
<table:table table:name="Foo" table:style-name="ta1" table:print="false">
exists in content.xml
This is a non-obvious failure which required unzipping the ods file and grep'ing
for "Foo" to find the name conflict.
