Concerning your proposal (a) it might be a challenge to insert the Notes information if there are several paragraphs or a bullet list (presumably in OOo 3.1). This might strongly interfere with the intention of the user to structure Notes information. (Assumption: The whole Notes information is inserted in - or between - the body text, changing its layout.) For a future design, we developed design proposals for (b) and (c) (see link). Could you please have a look at it and tell us if this fits your needs? If not, please describe your point of view "why" something is not sufficient.
