I was not able to reproduce exactly the same problem with the A4 size not
displaying the first slide however I performed the following steps and made the
following observations of similar behaviour.

Using the same presentation file but Windows XP SP 2 (OOO300m8 Build 9357)

1. Open the file by clicking on the document.
2. Select Format> Page>  
3. Change the Page size to A0 and click OK
4. The preveiw pane shows only 2 slides and text for Slide 6 (as shown in the
attachment)
5. If you move the slider for the preview pane, the outlines of the remaining
slides start to appear and if you resize it, all of the slides appear normally
in the preview pane. 
6. Once the slider has been resized, the behaviour is again normal with all
slides being visible in the preview pane. 
7. To recreate the problem you have to close Impress and then restart by opening
the file and following the steps as outlined above.  By simply resizing the
slider and reopening the file, there was no difference – I had to change the
page size. 
The following Page formats were also attempted with the following observations:
1. A1 shows the preview of 3 slides
2. A2 shows the preview of 4 slides
3. A3 shows the preview of 5 slides
4. A5 has all 6 slides visible

I also tried other presentation documents with multiple slides and experienced
similar behaviour. 

In Powerpoint 2007, I was not able to reproduce the problem. The preview pane
showed all of the slides. 

Other than moving the preview pane slider, there is one more action which will
display all slides normally. Following are the steps.

1. Open any Impress file by double clicking on the file. (Let’s say “File1”)
2. Open another impress file from  the menu “File > Open”  (Let’s Say “File 2”)
3. In the opened file “File 2”, change the Page size to A0 and click ‘OK’ – The
problem is reproduced.
4. Press “Alt +Tab” from the keyboard -> Impress file “File 1” is displayed.
5. Press “Alt+Tab” again from the keyboard -> “File 2” is displayed but the
problem has disappeared.
