Your document has mixed up chapter numbering and list numbering. That can not
work.
How to repair:
1. Edit > Find&Replace, extend dialog with "More Options" and check "Search for
styles".
2. Select style "Heading 1" in the search field.
3. Click "Find all". Now all headings at lever 1 are marked.
4. Click on button "Numbering on/off". Headings are still marked.
5. Open style and formatting window, tab paragraph styles. Double-click style
"default". Headings are still marked.
6. Double-click style "Heading 1".
Do similar for all levels which has wrong numbering.
If you are uncertain, whether the numbering is a list numbering or a chapter
numbering, go to Tools > Outline Numbering > Tab Numbering. Choose a level and
then select a number type which you do not use, for example circled numbers. All
headings of that level, which will follow the new type, have got outline
numbering, all others have got list numbering. Turn back number type to the
correct one.
