Press "F11" to open "Style and Formating" Dialogbox, use "Heading 1". 3. Put the cursor at the end of these words, "Insert -> Indexes and Tables -> Indexes and Tables...", unselect the "Protected against manual changes" check-box and left-click OK button on the pop-up "Edit Index/Table" dialogbox. 4. Press "Ctrl + A" in keyboard, right-click -> "Edit Index/Table", then left-click OK button on the pop-up "Edit Index/Table" dialogbox.
