I fixed the footnote problem long ago, but when I tested this issue, I realized
the footnote index in the bottom was duplicated for every line (paragraph)
instead for only being used for the first one.
This I have fixed now.
Thank you very much for this issue with that the nice test document making that
issue visible to me.
