Instead of 1. aaa 1. bbb 2. ccc 1. ddd one will start to get 1. aaa 1. bbb 2. ccc 1. ddd The tab stop sliders are not being moved appropriately to the next level. The tab arrow (non-printing character) goes from small and tight between number and text to larger, with space between the number, the arrow, and the text. The bullets and number dialog shows that it thinks proper indentation stops are still defined for the outline. No amount of reapplication fixes the issue. The only fix seems to be to change everything to default formatting and rebuilding the outline format, which may eventually start having problems again. I cannot recreate this in any deterministic way.
