To make it simpler and intuitive, I would like to see the spell checker deal
with this as follows.

1. Whenever you add a word in lowercase, then the spell checker should accept it
lowercase, with a capital first letter, and all uppercase.

2. Whenever you add a word with a capital first letter, the spell checker should
accept it both with a capital first letter and all uppercase, but not all
lowercase (because it could be a proper noun) -- unless you subsequently add it
all lowercase.

3. Whenever you add a word all uppercase, the spell checker should accept it
only all uppercase (because it could be an acronym) -- unless you subsequently
add it with a capital first letter or all lowercase.

This would save the spell checker from having to worry about punctuation.
