To reproduce, open a OOo document that has macros stored within the document
(not under My Macros or OOo Macros).

Select menu item Tools >> Macros >> Organize Macros >> OOo Basic...
Click "Assign..." button on OOo Basic Macros dialog
Select the "Events" tab on the Customize dialog
Click "Macro..." button

In the Macro Selector dialog that appears, notice how the box labeled "Library"
does not list the document as an available source for macros, only My Macros and
OOo Macros. As such, a macro that is embedded in the document cannot be assigned
to events for that document.
