In this specific example, on page 53 of this document (Figure 4-7) is a frame with a caption that is set to display As Character. Within this frame is an image. If you click "Send to Back" on the image, you'll see that there are three callouts that show up. After the document is saved and re-opened, however, these callouts disappear, reverting to being behind the image.
