The rtf opens as a blank page (WordPad opens the rtf correctly although without footer). The odt was opened then saved unchanged as doc (XP) which can be opened correctly. The doc was opened then saved unchanged as rtf, which opened correctly. I would attach the odt but it is not obvious how to do so.
