MRU->seasoned_geek: what build of OpenOffice do you run? Looking into the
meta-data of your document, it looks that your Linux distribution runs an old
developer build of OOo which wrote the odt file. Please use the official build
from OOo webpage to create valid odt files. The official OOo 3.1 build has the
ID "OOO310m11 (Build:9379)".
You can easily correct the missing Heading numbering; go to Tools.Outline
numbering and apply "Heading 1" style for first level and "Heading 2" for second
level.

MRU->OD: please have a look, OOo 3.1 is not able to interpret the Outline
settings; no style will be applied to the first two levels of the Outline
