When you add a form object in a spreadsheet (Calc) file and save it as a 
Microsoft Excel (.xls) document, any information entered in the "List Entries" 
field on the List Box (or Combo Box) Control Properties box are removed.

When you are in the document and add the form element, it works perfectly 
well.  After saving the document and closing Calc, the next time the document 
is opened, the List Box (or Combo Box) is empty.
