Here is the way to reach this bug: - Create a new Impress blank presentation, or pick an existing one - Insert - > Objects -> OLE Object - Choose "Other objects" - Selec one of the objects (for example a PDF object) - Select "Display as Icon" - Click on "Change Icon" - Select another icon from file - Click OK.
