Invalid issue:
1) 2 different problems in 1 issue.

2) a) it is so because a cross-reference can have many different source
Bookmarks, numbered paragraph... so it's convenient to group those objects in
the Insert Cross-Ref dialog, to be able to insert reference to whatever object
is possible.
What you would like in a button [Insert Reference to Bookmark] in a dialog which
is not modal (so you cannot change the cursor position to insert the reference
elsewhere).

Following your thoughts is to ask "Why are the targets of a hyperlink grouped in
the hyperlink dialog?".

2) "Set" is a synonym of "define", please discuss this on the
[email protected] mailing list.
