My attempt to reproduce:
1. Created new DRAWING and "Saved as" somewhere in "My Documents" as
.....\111763_relativepath\Document\Document.odg
2. Inserted Smiley image and exported that to
.....\111763_relativepath\Drawings\2D-Images\PNGSmiley.png
3. Menu 'Insert -> Picture -> From File -> Browse "PNGSmiley.png"'
4. Inserted same picture using insert button from toolbar
5. saved document
6. closed
7. reopened from "Recent documents"
expected: all 3 smileys visible
actual: as expected
10. Closed document (close window button), Exit from OOo
11. Restarted OOo from WIN Start Bar
12. Opened sample document from "Recent Documents"
expected: all 3 smileys visible
actual: as expected
13. closed document (close window button) and Exit
20. Renameed path "111763_relativepath" to "111763_relativepath_"
21. Started OOo
22. Opened "Document.odg"
expected: all 3 smileys visible

Please check your OOo version and create help screenshot, I doubt that
"OOO32m21" is correct. Your build number leads to a very very obsolete OOo
version.
Please contribute information concerning source of your OOo!
Unfortunately your sample document doesn't prove anything, because we can't see
the path of your sample document. Please create a test kit like mine.
Do you also see that problem in CALC, WRITER?
