I have a document I brought in, and what has happened is that some of the Table of Contents entries have "collapsed" while others have not. What I mean by "collapsed" is the item that is marked as a table of contents entry becomes unmarked and the TOC entry is a highlighted half-space appearing immediately before the entry. The items that were in the TOC are still there, the TOC remains correct, and updating the TOC via right click still works ok, so that's not a problem - in fact, it's not bad because I can have items as unformatted in the TOC but mark them as underlined in the text - the problem is that this behavior is not consistent, new entries are remaining highlighted and do not collapse. I can live with it either way, but having some collapsed and others not collapsed is confusing.
