Work around Run a standard spell check first. (F7 or Tools >> Spelling). This will display the [Spelling] dialog box. When the spell checker finds the word that you want to add to the dictionary, click the [Add] button in the [Spelling] dialog box. Thereafter, the [Add] option in the right-click context menu should work as expected. That is, selecting the [Add] option and then selecting a dictionary will then actually add that word to the dictionary, and added words will no longer be underlined in red. I suspect that the vast majority of users run a spell check before using the right-click context menu, and thus never experience this problem. But OO would be a better product if the context menu worked correctly first time.
