Issue is back. Unable to modify document without freezing and crashing. I have
noticed excess disk activity when document freezes up. I have also noticed that
it has something to do with the Index and the headings locations and on which
page. Insert a few 'Returns' so that an Index Headings moves to the next page
brings the crash on. Adding a Heading, which is picked up by the front Index,
also brings the crash on.
By working without the Index seems to clear things up until you try to re-create
the Indexes. The new index won't pickup the headings.
