I may have a solution for this.  If you wish to have a copy of the template I

I have just composed a basic template to solve this problem, and it is attached.
 It works fine and it is of type ".ott" -- Open Document Template, from OOo
1.9.104 [v2 BETA].

It would be nice if this (or something similar) would be included in the final
OOo v2 release, but it is probably too late for that.  Barring such a "virgin
birth" type of miracle on that, you can save this to your hard drive, then
Import it into your templates.  Let me know if you have any questions on either
how I did this, oor how to incorporate it into your own OOo install.
