If you open it up in Adobe Acrobat Professional 6.0 and select "Document > Pages > Extract" to extract page 1 of the document to a separate PDF, Adobe puts up an error message that reads "An incorrect structure was found in the PDF file". I'm also attaching the resultant PDF.
