I will occasionally have a user respond to a ticket via email and try to 
provide an attachment, but this just seems to disappear into the ether.

This should either be added to the post as an attachment, or have a message 
sent back to the user telling them that the file wasn't attached and that they 
should visit the ticket/discussion directly.
