Have you unmuted and raised the volume of the CD input with a mixer? And is 
there a cable between the analog out of the CDROM and your soundcard ? Does
the CD 'seem' to be playing ?

Well, KDE uses the arts sound daemon, realplayer should use artsd to mix
it's audio stream with other audio streams. Maybe you can select an 
appropriate output plugin ?
