I had numerous times when this occurred on my DELL laptop - maestro3. I thought I'd figured it out that it had to be "Master Mono" channel unmuted and turned up. Next time - that wasn't it. So I unmuted every channel, turned up all the volumes, then tried arec/aplay for a quick sample, each time muting a channel. Turns out I had to have "Mic Boost" channel unmuted/turned up.
