Can you send along the .eeprom file so we can take a look at the data?
It certainly has some 'interesting' values in the flight overview you
provided.

You can take a look at the recorded acceleration data to see if the unit
experienced any really high g forces which might cause it to fail, and
you can look at the sensor values using 'monitor idle' mode to see if
they're reasonable.
