In addition to the other suggestions of fsck, making sure your disk
geometry/partitioning is correct, etc, this might also indicate an
active file system.  Dump saved this inode away early in the processing
as needing to be backed up, but when it got to actually dumping it the
disk block pointers had been released, reallocated, etc, and pointed
off into nowhere.  There isn't a whole lot you can do about this.

I would point out that if this is the case (and I'm not all that sure it
is), the other (non-Linux) dump programs I'm aware of do not consider
this a fatal error, so you might want to pass that back to the Linux
dump maintainers.
