I agree with Gerhard that this sounds like a hardware problem. But in
addition to the tape drive itself, I'd also check the other usual
suspects:
* Make sure any DIP switches on the drive or controller are where
they belong and firmly locked into position.
* Make sure the cables are firmly attached.
* If this is SCSI, make sure there is one and only one terminator on
the bus and that it's at the end of the chain.
* Make sure you have followed SCSI specs on total bus length (which
must include "internal" cable lengths), wide vs. narrow ordering, etc.
* Make sure the controller is firmly seated.
