This looks like a classic data sensitive hardware problem.  When presented
with a particular data pattern, some portion of the hardware is messing
up.

In order, the things I would try replacing are:

  SCSI termination (and make sure you don't have two terminators on the
  chain or that you don't have zero).

  The cable.

  The drive.

If the terminator is bad (or missing, or too much), you can get
"reflections" that cause all sorts of havoc.

If the cable is not properly shielded, you can get crosstalk.

Or the drive might just be toast.
