Show these settings: $final_spam_destiny, $spam_quarantine_to, $sa_kill_level_deflt and $sa_tag2_level_deflt. Are you using @spam_kill_level_maps or @spam_quarantine_to_maps ? In other words, are you using per-user or per-domain settings? Was the email addressed to only one recipient, or more than one? What I am getting at is this: if you have your quarantine configured to send mail to a mailbox, maybe the mail was addressed to that mailbox.
