Me too.
The general idea is to list the IP addresses or network address of
computers that are allowed to relay mail through your server in both
mynetworks (Postfix) and @mynetworks (amavisd-new). It is important
that you do NOT list any IP addresses/networks that should NOT be able
to use your server as a relay. That can be a very bad thing.
But you are accessing your server through a dial-up connection?

So the desktop computer you are sending mail from is not on the same
network as the server. I'm sure it would be a bad idea to list the
dial-up network's address, even with authentication.
Question is, would it do any good (or harm) to list 192.168.0.0/16
along with 127.0.0.0/8 ? I'm not sure myself.
I'm not certain, but the $MYNETS policy bank may not be right for
this situation. The computer you are sending from needs to be on the
same network as the server. It appears this is not the case. Is this
correct?
Just curious, it appears you have been given 512 IP addresses by your ISP.
Is this also correct?
