While it might be fun to modify clamav's code for no good reason, you could instead create your own signature. Instructions can be found in the clamav-users mail archive, and in

I've checked and i'm pretty sure spamassassin do not support content
filtering. It is not suppose to open the attachment.

Not support content filtering?  What other purpose does spamassassin have?
Of course SpamAssassin can scan text attachments.
