Milter has some advantages (being invoked during original mail session it is able to reject it), and it has several disadvantages. Milter is useful for lightweight content filters (like graylisting, RBL and such), but is not appropriate for heavyweight mail checks with SpamAssassin (with a possible exception of small/home sites with low mail traffic). More in README.sendmail-dual.
