Milter has some advantages (being invoked during original mail session
it is able to reject it), and it has several disadvantages.
Milter is useful for lightweight content filters (like graylisting,
RBL and such), but is not appropriate for heavyweight mail checks
with SpamAssassin (with a possible exception of small/home sites
with low mail traffic). More in README.sendmail-dual. The same tradeoff
holds in the Postfix world: smtpd policy delegation vs. post-queue
content filters.

  A recommended sendmail setup is described in file README.sendmail-dual,
  which describes a dual-MTA setup. The sendmail milter setup as described
  in README.milter works as well, but with some functionality limitations.


Admittedly the README.milter does not contain such a warning.
I'll add the following in that file, thanks for the suggestion:

NOTE:

  which describes a dual-MTA setup. The sendmail milter setup as described
  in this file works as well, but with some functionality limitations.
  Please see the introduction section in README.sendmail-dual when deciding
  between milter setup and a dual-MTA setup.
