Messages larger than $sa_mail_body_size_limit are not sumitted to SA for checking, the default limit is 200 kB. Have you increased this setting substantially? SA scanning time goes rouggly linear with message size, and (since very large spam is quite rare), it makes sense to not waste time at spam-scanning large messages.
