Yes, using Petr Rehor's amavisd-milter is a must if you need dynamic
headers (like X-Spam*) to be inserted, or Subject to be edited (inserting
***SPAM***).  I'd recommend it even if you don't.

Yes, sensible detailed reports in the header ($sa_spam_report_header=1)
are possible with 2.4.1 and Petr Rehor's milter.

Detailed reports in the mail body are not possible.
(unless you switch to Postfix, which opens a choice
for a working $defang_spam=1)
