The following operation failed:

  BerkeleyDB::Queue->new(
   -Filename=>'cache-expiry.db',
   -Flags=>DB_CREATE, -Env=>$env, -Len=>15+1+32);

I can only guess why BerkeleyDB says the file already exists.

Perhaps the previous version of file 'cache-expiry.db'
is not owned by vscan or too heavily protected and can not be
deleted and replaced by a new database on startup. Check also
the directory protections, it's location is $db_home.
